1. Participation fees (with the Early bird Discount) for football & cheer are $300 for the first child, $250 for the second child, $225 for the third child and $200 for any additional child from the same family. These fees cover the cost of: facility usage, uniform and equipment use, insurance, officiating, and light rental. 2. Early bird discount ends April 30th. ($25 discount) 3. I agree to pay $100 for the raffle ticket fundraiser. Prizes TBD. Raffle tickets can be picked up during the first week of tryouts. I have the OPTION of selling my raffle tickets to compensate my initial $100. Raffle tickets stubs can be turned in at the Equipment shed during any practice up to the football rally. 4. I agree to pay the $100 work shift fee. This fee must be paid at the time of registration. This fee will replace the work shifts required in previous years. (However, if you would like to work your shift hours and receive a refund, you will be able to sign up for shifts in August) 5. Each football participant will need to purchase a pair of football cleats, black socks, and practice jersey. Football players agree to return all equipment loaned to them or pay for the cost of that equipment if it is lost, stolen or not returned. 6. Equipment Turn-In. The equipment turn-in date will be scheduled after the last DOJGE play-off game. This will allow players and cheerleaders to attend any/all DOJGE play-off games with their jersey's or uniforms. It is the parents/players responsibility to turn in all of their equipment on this date. If you are unable to make this date, it is the parents/players responsibility to contact the Equipment Coordinator, to coordinate a turn-in date and location. Any equipment NOT turned in will be deemed lost and your $300 equipment holding check will be cashed. The total cost of the equipment will be owed to DOJGE immediately. 7. I agree to give a $300 holding check for lost or damaged equipment. This check will not be deposited. If I don't turn my equipment by the turn-in date, I will forfeit the $300 holding check for equipment and still owe for the cost of the equipment that is not returned. 8. Returned Checks. There will be a $25.00 charge on all returned checks 9. Both football players and cheerleaders must obtain a physical at their own cost on or after April 15 (of the current year) . This must be completed on the current year SAC Medical Release form signed AND STAMPED by a licensed doctor prior to the first practice or cheer camp. 10. If my child is cut from the football team during try-outs I will receive a full refund. If my child decides to quit for any reason during tryouts, I will receive a full refund minus $25 for administration costs and $100 for raffle tickets. I understand that no refunds of the above fees will be given after the last day of try outs. 11. Payment in full is due by July 13, 2017. Any football player or cheerleader that has not paid or not completed paperwork, will not be allowed to tryout or practice.
In the event that a team has more than thirty-five players, it will be the responsibility of the coaching staff to assemble their respective teams and that decision will be reached through consensus of the staff. While an athletes talents and skills are obviously an important element in considering whether or not a player is selected to a team, they are not the overriding factor in the selection process. Accordingly, selection guidelines have been mutually established by both the DOJGE board and the head coaches of the association. These guidelines are as follows.
SAFETY: If the coaching staff determines that a player is potentially at risk of injury or appears to be injured at anytime, it will be within the coaches discretion to opt not to select that player to a position on the team. Each head coach will consult with the coaches coordinator and board president before any safety cuts are made.
DEL ORO HIGH SCHOOL SPHERE OF INFLUENCE: For new players this year (2016) to the Del Oro Jr. Golden Eagles, any child who is not attending school within the Del Oro High School sphere of influence will be subject to first round cuts regardless of any other circumstances including talent. Current 2015 players are grandfathered into the program and not subject to this criteria.ATTITUDE/EFFORT: The coaching staff will greatly weigh the players dedication toward his work ethic and desire to compete. ATTENDANCE: It is imperative that each player attend every practice during the tryout period. A talented player who does not attend these practices will greatly reduce his chance of securing a position on a squad. Missing one or more of these practices does not require that the child be cut, however, it does play a significant part in the coaches selection. To be clear however, if your child makes all of the practices and another child does not, that does not ensure your child a higher place on the team. It is about energy, effort AND skill.PARENTAL CONDUCT: As parents, we are expected to lead our children by example. Our association is committed to promoting an atmosphere of respect and cooperation. It is our intent not to subject our players to acts of misconduct perpetrated by adults. Therefore, failure to adhere to SAC Adult Code of Ethics form shall place player selection opportunity at risk.
1. Sierra Athletic Conference (SAC) is a group of member associations that has agreed to play together only. 2. Membership within Del Oro Jr. Golden Eagles (DOJGE) starts when you are signed up and have paid in full. Membership expires on November 30, 2016. 3. I/We have read, understand and agree, as parent/guardian of the below named child to abide by the role of the parent and players code of conduct and assume the absolute financial obligation for my/our child to participate in this youth program. 4. The child named below has read, understands and agrees to abide by the player code of conduct. 5. The parent/guardian of the below named child, do hereby give my/our approval for participation in Del Oro Jr. Golden Eagles (DOJGE)/Sierra Athletic Conference (SAC) activities for the current season. I/We assume all risks and hazards to this participation for any claims arising out of injury to the above named child, including, but not limited to, transportation to and from such activities. I/We hereby waive, release, absolve, indemnify and agree to hold harmless Del Oro Jr. Golden Eagles (DOJGE)/Sierra Athletic Conference (SAC), the league, local team, organizers, managers, coaches, supervisors, participants, person providing transportation and any organization this youth football program may be affiliated with. 6. In executing the foregoing release. I/We acknowledge that I/We understand that our personal medical/dental insurance will remain the primary carrier, and that insurance offered through this program is secondary in nature and is subject to an annual deductible by the carrier. It is understood that any claim for injury arising out of my/our child's participation must be reported to the designated association official within 30 days of the date of injury. It is also understood that the proof of loss must be completed in full and filed within 60 days of receipt by Del Oro Jr. Golden Eagles (DOJGE)/Sierra Athletic Conference (SAC). All monies I/We have paid to the team do not constitute payment of insurance coverage. I/We do indemnify Del Oro Jr. Golden Eagles (DOJGE)/Sierra Athletic Conference (SAC), the association and the insurance carrier should there be statement(s) by anyone that is in contradiction. I/We attest I/We has read and understand the terms of this contract and any disclosure information required. 7. I/WE understand that if player/cheerleader has not had their physical examination they will not be allowed to participant in any Del Oro Jr. Golden Eagles (DOJGE)/Sierra Athletic Conference (SAC) activities. I/We hereby grant authority to a qualified physician to administer such medical treatment, as said physician deems necessary under emergency circumstances. 8. Del Oro Jr. Golden Eagles (DOJGE)/Sierra Athletic Conference (SAC) players and cheerleaders can participate in other organized sports as long as it does not interfere with any of the Del Oro Jr. Golden Eagles (DOJGE)/Sierra Athletic Conference (SAC) practices and games. Attendance is necessary to continue involvement. School sports are encouraged along with any type of program that would help your child excel in the sport of Football and Cheerleading. 9. I understand that all football/cheer equipment given to my child will be turned when the association requests or has a equipment turn-in. If there are any questions please contact your local association. 10. I/We have read and understand fully the provisions of this consent/release authorization, and I/We have voluntarily signed it.
Release of Liability
In consideration of my child/ward, being allowed to participate in any way with the Del Oro Jr. Golden Eagles that is a member of the 2016 season with the Sierra Athletic Conference (SAC) related events and activities, the undersigned acknowledges, appreciates, and agrees that:1. The risk of injury to my child/ward from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,2. I FOR MYSELF, SPOUSE, AND CHILD/WARD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my child/wards participation; and,3. I willingly agree to comply with the programs stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child/wards readiness for participation and/or in the program itself, I will remove my child/ward from the participation and bring such attention of the nearest official immediately; and, 4. I for myself, the legal guardian of the said child, my spouse, my child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Del Oro Jr. Golden Eagles, and any member organization of the Sierra Athletic Conference (SAC) league that is a member of this said year, its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (Releases), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child/wards involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE, to the fullest extent permitted by law.5. I, the legal guardian of the said child, for myself, my spouse, my childs/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releases from any and all liabilities incident to my childs/ward involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

1. I hereby pledge to provide support, care, and encouragement for my child participating in youth sports by following this Parents Code of Ethics: 2. I will encourage good sportsmanship by demonstrating positive support for all players, cheerleaders, coaches, and officials at every game, practice or other youth sports events. 3. I will place the emotional and physical well being of my child ahead of my personal desire to win. 4. I will insist that my child play in a safe and healthy environment. 5. I will require that my child's coach be trained in the responsibilities of being a youth sports coach and that the coach upholds the Coaches Code of Ethics. 6. I will support coaches and officials working with my child, in order to encourage a positive and enjoyable experience for all. 7. I will demand a sports environment for my child that is free from drugs, tobacco and alcohol and will refrain from their use at all youth sports events. 8. I will remember that the game is for youth not adults. 9. I will do my very best to make youth sports fun for my child. 10. I will ask my child to treat other players, cheerleaders, coaches, fans and officials with respect regardless of race, sex, creed or ability. 11. I will help my child enjoy the youth sports experience by doing whatever I can, such as being a respectful fan, providing transportation, my child attending practices, and keeping equipment clean.
In registering for Del Oro Jr. Golden Eagles, I understand that my child's picture and/or name may be used in a program, newspaper or on a website.

I understand the sport my parents are signing me up for has risks that could result in injury. My parents have explained this to me and I choose to accept those risks by checking the box below.
